Responsibilities
Assessment Coordination
- Schedule and conduct person-centered assessments with individuals and families in their homes or community settings across New York.
- Utilize conversational interviewing techniques to prioritize individual preferences and needs.
- Arrange professional interpreter services as needed.
Documentation & Quality Assurance
- Complete assessments using specialized software, adhering to accuracy and coding guidelines.
- Document all activities and meet productivity goals (24 face-to-face assessments per month).
- Ensure assessments meet quality standards in completeness, timeliness, and individual participation.
Skills/Experience
- Limited experience at consultancy preferred;
- Great interpersonal communication skills;
- Keen eye for spotting data trends;
- Great analytical skills;
- A keen grasp of information technology;
